How to Enrol

All families using the service must complete an online enrolment, listing emergency contacts and medical information. A once-off enrolment fee of $20 for new families will be added to your account after your first booking is made. Enrolments, bookings and cancellations may be completed through Fullybooked via the buttons below.

Prior to your Childs first day of attendance, please provide the service with a completed Safety and Risk Minimisation Plan, Health Support Agreement, Care Plan and a Medication agreement if medications are to be administered whilst at OSHC. All medications (including ointments and creams) must be prescribed by a medical practitioner, be in the original container with original pharmacist labels and clear instructions. All labels must be legible and undamaged, and the medication must still be within its expiry date. Please find forms below.
